Adding users to our domain

I'm using the User Management Resource Administrator to design a form to
handle account creation for my organization. However, I can't get the
form to show up inside my help desk's client. I have checked permission
on the shares and everything else, but it still won't show up in the
list. I'm connected to the right UMRA service on the network, and
logged in as an administrator (not the same acct used to create it).
Even if I log in with the account that created it, I still cannot see
the form.

Any help or ideas would be appreciated.
