Well, I have an answer to my own question after a lot of trial and
error. Turns out the culprit is the Legal Notice Dialog Box before the
logon screen.

If the Legal Notice Box is visible, a Windows XP-controlled monitor
will not sleep reliably.

I solved the problem by moving our Legal Notice text to the logon
screen. How? Use the LogonPrompt entry in
HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows
NT\CurrentVersion\Winlogon to add any text above the user name and
password on the logon screen.
