In XP Pro is there a way to force all programs to open up in a maximised
window?

Most programs remember. But if you wish, you can Right Click
the Shortcut/Icon and Left Click Properties. In the Details Box,
toggle the Run field to Maximize, Apply & OK.

I have done this where possible but for instance, when I arrived at this page
direct from the email notifying me of your answer it did not open maximised.

There are many "Hooks" and calls within applications that I'll have to
admit I don't know the inner workings or how to explain them. Shell
extensions and the like may have hard coding or fixed parameters it
isn't easy to modify. Perhaps someone with a better understanding of
your issue(s) will post and help you along.

Use your mouse, drag the window to full size and then click File and Close.
This works for IE, OE and Windows Explorer.
