Try removing both instances of the card and reinstalling the card's drivers
after a reboot. You may have to run in Safe Mode to see the two entries in
the Device Manager. Remove them both.

Otherwise, this may be a resource conflict of some kind. I find that my USB
camera conflicts with my USB scanner for reason, even though there should be
room for both devices.
